Where a client provides parking instructions, the client confirms they have authority to authorise the parking arrangements provided. Any parking charges, penalties or enforcement costs arising from following the client's parking instructions may be recharged to the client*

I agree not to attend my appointment if I have any contagious symptoms including (but not limited to):

• fever or raised temperature (37°C+)

• cough or respiratory infection

• loss of taste or smell

• any contagious illness within the last 24–48 hours

I will inform the therapist immediately if any symptoms develop before the appointment.*

I understand that inaccurate or incomplete information may result in treatment being refused or discontinued for safety reasons.

Treatment Suitability & Therapist Discretion

All treatments are subject to professional consultation and suitability assessment.

The therapist reserves the right to:

• refuse treatment

• modify treatment

• or recommend an alternative service

if it is deemed unsafe, unsuitable, or outside professional scope of practice.*

Certain treatments may also be subject to training limitations, insurance requirements, safeguarding considerations, and treatment-specific suitability assessments.*
